This is intended to be a legally binding document between the guest and the Just For Jesus Challenge Homeless Outreach Ministry.

I __________________________ am in agreement that if I leave the Just For Jesus Challenge Homeless Outreach Ministry property unattended without approved staff / assistant volunteer / volunteer / or person, that I am requesting that the DuBois Barracks of the Pennsylvania State Police be called immediately and I request that they immediately pick me up and transport me to the nearest jail for my own safety as a free man or woman, due to the stigma given to me by the legal system; and / or violating my parole by not abiding by the guidelines of the Just For Jesus Challenge Homeless Outreach Ministry.

Therefore, I am requesting my constitutional right to be protected from the possible threatening and intimidating action or harm that may be brought upon my person by the community, because of the labeling that I have been stigmatized with.
